New telecommunications carriers must register with the Federal Communications Commission (FCC) when they begin to provide service by filing the FCC Form 499-A (Annual Telecommunications Reporting Worksheet) with USAC. Registration information includes contributor identification information, regulatory contact information, agent for service of process, FCC Registration Number, and FCC contact information (reported in Blocks 1, 2-A, 2-B, and 2-C of the FCC Form 499-A). New carriers must identify an agent for service of process within 30 days of providing service. Upon registration, all carriers must update their information within one week if their contact in Washington, D.C., (the D.C. Agent) changes in Block 2-B. FCC Form 499-A is due in April each year regardless of contact changes.
Companies cannot register by filing only an FCC Form 499-Q (Quarterly Telecommunications Reporting Worksheet), the quarterly projection form. If an FCC Form 499-Q is submitted as the first filing, USAC will contact the company about filing an FCC Form 499-A before it can be considered registered.
