If you, as a contributor, owe a debt to the universal service fund (USF), you have an opportunity to request a written installment payment agreement (including a promissory note) to pay the full amount of the debt. In order to request an installment payment agreement, among other things, you must first provide evidence that demonstrates financial inability to pay the debt in one payment. Your claim of financial inability to pay in one payment is subject to verification (see 31 CFR §901.8) and if your request is approved for further processing, you will be required to execute a written agreement suitable to the Federal Communications Commission (FCC).
You should be aware that payment plan agreements regularly impose a number of obligations on the debtor including additional interest and administrative fees, and possible audit obligations.

If you receive a collections letter from USAC or the FCC notifying you of your right to request a payment plan agreement, it is important to act promptly and within the time period stated in the letter in order to preserve your right to request a payment plan.
