Information on requirements for the Service Provider Identification & Contact Information (Form 498)
The Service Provider Identification Number and Contact Information Form (Form 498) was revised in November 2009 to encompass all four support programs. USAC encourages all providers and recipients to review the instructions prior to completing the form. The instructions for completing Form 498 are available in PDF format.
First-time applicants will be assigned a Service Provider Identification Number (SPIN) by USAC. The form will be used to collect the following information: service provider name, address, phone numbers, e-mail addresses, contact names, billing, and collection information. USAC will use this information to administer the billing, collection, and disbursement operations of the federal universal service programs.
