Rural Health Care Tools:
Step 1: Determine Eligibility
Health care providers (HCPs) participating in the Rural Health Care Program must be eligible and must select eligible services and providers in order to receive discounts.
Before beginning the application process, it is important to confirm eligibility. The Eligible Health Care Providers page can help you determine eligibility.
In general, participants in the program must be rural and public or non-profit health care providers. A complete list of eligible entities is available on the Eligible Health Care Providers page.
In 2004, the Federal Communications Commission expanded the definition of "Rural" for participants in the Rural Health Care Program. To determine if your location is considered rural, please select Rural Health Care Search Tools on the left side of this page and then go to Rural Eligibility Search.
For a list of eligible services, go to the Eligible Services.
If you have any questions, please contact USAC's Customer Service Support Center at 1-800-229-5476.
