Step 2: Submit a Service Request
Posting the Description of Services Requested and Certification Form (FCC Form 465) is the initial step a health care provider (HCP) must take in order to benefit from the Rural Health Care Program.
Rural Health Care support allows eligible HCPs to purchase certain services at reduced rates. Filing an FCC Form 465 enables a health care provider to request bids for the provision of telecommunications or Internet services from service providers, and certifies to USAC that the HCP is eligible to participate in the Rural Health Care Program.
HCPs that have previously posted an FCC Form 465 on the USAC website can register for electronic certification (e-certification). E-certification allows an HCP to electronically sign the Form 465 so it can be immediately posted on the USAC website after review.
After the HCP submits a Form 465, USAC will post the completed form on the USAC website, which provides information about the HCP's service needs to service providers that may bid to provide the services. Health care provider service requests can be searched using the Search Service Requests tool.
