Step 5: Other Documentation Needed
A Funding Request and Certification Form (FCC Form 466) and/or an Internet Service Funding Request and Certification Form (FCC Form 466-A) submitted with e-certification requires supplemental documentation to be sent to USAC by mail.
Forms with missing or incomplete information or documentation will not be processed.
Supporting documentation includes, at minimum, a copy of a bill that shows the cost of the service. In the case of a large state network or consortia, it may be necessary for the participant to send a network diagram. A copy of the contract should be sent to USAC, if applicable. All information can be mailed, but where possible USAC requests that applicant use the fax number below to submit documentation.
