Step 7: Submit Confirmation of Service

The HCP must complete and submit the Connection Confirmation Form (FCC Form 467) to USAC.

After USAC sends the HCP and service provider the Funding Commitment Letter, and the HCP's service from a service provider begins, the HCP must submit the Connection Confirmation Form (Form 467) to USAC to receive its discounted services. USAC cannot process the FCC Form 467 unless a Funding Commitment Letter was issued. The FCC Form 467 is available for e-certification.

An HCP must submit one FCC Form 467 for each Funding Request and Certification Form (Form 466) and/or Internet Service Funding Request and Certification Form (Form 466-A).

The Form 467 must also be used to notify USAC if and when the HCP has discontinued the service (or the service was turned off), or that service was not (or will not be) turned on during the funding year.
