A revised version of the FCC Form 498 is now available. With this revision, the term "Service Provider Identification Number," or "SPIN," has changed to "498 ID."
The FCC Form 498, previously a form used exclusively by service providers to provide contact and banking information to USAC, now includes a block for Schools and Libraries Program applicants who have selected to use the BEAR invoicing method. On the revised form, applicants of the Schools and Libraries Program can provide contact and banking information to receive billed entity applicant reimbursement (BEAR) payments directly from USAC, rather than through their service provider.
To reflect the change of the FCC Form 498 from a service provider-only form to one that is also used by certain program applicants, the form's name, the "Service Provider Identification Number and General Contact Information Form," is now, as of December 1, the "Service Provider And Billed Entity Identification Number and General Contact Information Form," and the number assigned to each payee will now be called a "498 ID," instead of a "SPIN."
Only one section on the FCC Form 498 has changed for service providers. In Block 12, the Schools and Libraries Program section, there is no longer an option for service providers to select BEAR payments because applicants will now receive payments directly from USAC, should they choose this payment option. The rest of the form is unchanged for service providers.
