The Federal Communications Commission adopted an order (FCC 06-94) June 21, 2006, modifying the Universal Service Fund (USF) contribution methodology. This order will affect revenues reported beginning with the August 2006 FCC Form 499-Q filings for wireless carriers and interconnected Voice-over-Internet Protocol (VOIP) providers. The proposed contribution factor for the third quarter (July-September of 2006) is 10.5%

Public Notice On June 9, 2006, the Wireline Competition Bureau released the attached Public Notice announcing that the proposed universal service contribution factor for the third quarter of 2006 will decrease to 0.105 or 10.5 percent. In the FCC Report and Order and Notice of Proposed Rulemaking of June 27, 2006 (FCC 06-94), the FCC implemented revisions to the FCC Form 499-Q form due August 1, 2006, and its accompanying instructions.  If you wish to file the August Form 499-Q, you may do so using the current form (dated May 2006, OMB #3060-855) and report revenue as directed in the FCC Report and Order and Notice of Proposed Rulemaking of June 27, 2006 and the proposed revised FCC Form 499-Q and FCC Form 499-Q and 499-A instructions included in that order. On June 27, 2006 the Federal Communications Commission released a revised FCC form 499A and instructions.  This form is to be used by companies that are now required to contribute as indicated in The FCC order FCC 06-94 modifying the USF contribution methodology adopted on June 21, 2006.  This revised form should be used for new company registration only.  Revisions to previous filings should be filed using the FCC Form 499A that was originally posted in March 2006.  The instructions have been updated to reflect changes to the reporting requirements, and provide additional information for interconnected VOIP carriers.  Contributing companies are encouraged to review the updated 2006 499A instructions as well as the revised 499Q and instructions for important changes.  The updated form and instructions can be found on the USAC forms page.  Please contact Customer Service at 888-641-8722 with any inquiries.

The 2006 FCC Form 499A is due April 3, 2006

All carriers must have a certified form to USAC by that date. USAC stronly encourages online filing of the form. You may login to the USAC e-filing system or download the form by visitng the forms section of our website here.

On Wednesday, January 18, 2006 the Federal Communications Commission published a notice in the Federal Register at 71 FR 2937-01, 2006 WL 118099 (F.R.), inviting comments on a current proposed revised FCC Form 498, and on current proposed revised instructions for FCC Form 498. Upon completion of the comment period, the Commission proposes to submit revisions of both documents to Office of Management and Budget for approval. FCC Form 498 is the Service Provider Identification Number and Contact Information Form.

Comments on the current proposed draft form and current proposed draft instructions will be accepted by the Commission until April 1, 2006, and may be directed to the following individual: Judith B.
