Error Messages Relating to Your Eligibility

Proof of Federal Assistance Programs

If USAC needs more documents to show that you participate in a federal assistance program, you can show proof by uploading a copy of a document listed below to the National Verifier online portal or by mailing it to the Lifeline Support Center.

All documents must show that you are currently a participant of the Lifeline-qualifying program.

If you participate in Supplemental Nutrition Assistance Programs (SNAP), submit a copy of one of these:

  • An approval, benefit, or verification letter that includes:
    • Your name or your child or dependent’s name that participates in SNAP
    • Program name
    • Effective date
  • A benefit statement that includes:
    • Your name or your child or dependent’s name that participates in SNAP
    • Program name
    • Effective date
  • An unexpired SNAP card (only in Montana and Wyoming) that includes:
    • Your name or your child or dependent’s name that participates in SNAP
    • Program name
    • Identification or card number

If you participate in Medicaid, submit a copy of one of these:

  • An approval, benefit, or verification letter that includes:
    • Your name or your child or dependent’s name that participates in Medicaid
    • Program name
    • Effective date
  • A benefit statement that includes:
    • Your name or your child or dependent’s name that participates in Medicaid
    • Program name
    • Effective date
  • An unexpired state or federal Medicaid card that shows:
    • Your name or your child or dependent’s name that participates in Medicaid
    • Program name
    • Identification or card number
  • A health care card that includes:
    • Your name or your child or dependent’s name that participates in Medicaid
    • Program name
    • Policy or health care policy number
    • Indication of state or federal Medicaid participation

If you participate in Supplemental Security Income (SSI), submit a copy of one of these:

  • An award letter (from the U.S. Social Security Administration) that includes:
    • Your name or your child or dependent’s name that participates in SSI
    • Program name
    • Effective date
    • Benefit information
  • A check stub from the U.S. Social Security Administration that includes:
    • Your name or your child or dependent’s name that participates in SSI
    • Program name
    • The date

If you participate in Federal Public Housing Assistance (FPHA), submit a copy of one of these:

  • An award letter that includes:
    • Your name or your child or dependent’s name that participates in FPHA
    • Program name
    • Effective date
    • Benefit information
  • A Public Housing Assistance lease agreement that includes:
    • Your name or your child or dependent’s name that participates in FPHA
    • Program name
    • Benefit information
  • If you do not have one of these documents, call or visit the office that approved your public housing and ask for them. Find an agency at the HUD PHA Contact Information page.

If you participate in Veterans Pension or Survivors Benefit, submit a copy of one of these:

  • A pension grant letter that includes:
    • Your name or your child or dependent’s name that participates in Veterans Pension or Survivors Benefit
    • Program name
    • Benefit information Payment start date
  • A Cost of Living Adjustment (COLA) letter that includes:
    • Your name or your child or dependent’s name that participates in Veterans Pension or Survivors Benefit
    • Program name
    • Benefit information
    • Effective date
  • A Survivors Benefit summary letter that includes:
    • Your name or your child or dependent’s name that participates in Veterans Pension or Survivors Benefit
    • Your claim number
    • The related veteran’s number
    • Benefit information

If you participate in Bureau of Indian Affairs (BIA) General Assistance, submit a copy of one of these:

  • An award letter from the national or regional BIA office
  • A check stub from the BIA that includes:
    • Your name or your child or dependent’s name that participates in BIA General Assistance
    • Program name
    • The date

If you participate in Tribally-Administered Temporary Assistance for Needy Families (TANF), submit a copy of one of these:

  • An award letter that includes:
    • Your name or your child or dependent’s name that participates in Tribal TANF
    • Program name
    • Effective date

If you participate in Food Distribution Program on Indian Reservations (FDPIR), submit a copy of one of these:

  • An award letter from an Indian Tribal Organization (ITO) or another state agency that includes:
    • Your name or your child or dependent’s name that participates in FDPIR
    • Program name
    • The start and end dates of the award period
    • The telephone number of the Food Distribution Program Office
    • The name and address of the person to contact for additional information
  • A benefit card
  • A letter from the administering agency that states your participation in FDPIR

If you participate in Tribal Head Start, submit a copy of one of these:

  • A letter that should include:
    • The name of your child who participates
    • Program name

You may submit valid program documents that are not on this list. The review process may take longer if you show other documents.

Proof of Income

If USAC asks you for additional proof that your income qualifies for Lifeline, you can show proof by uploading a copy of a document listed below to the National Verifier online portal or by mailing a copy to the Lifeline Support Center.

All documents must include your name or your child or dependent’s name.

If your income is currently at or less than 135% below the Federal Poverty Guidelines, submit a copy of one these:

  • Prior year’s wage and tax statement or W-2
  • Prior year’s personal tax return or Form 1040 with the your name and address
  • Employer paycheck or pay stubs with the your name, address, and payment amount
  • Benefit or statement letter issued by the Social Security Administration (SSA) with your name, address, claim number, income, and effective date
  • Retirement/pension statement with the your name, date, and payment amount
  • Tribal letter of participation in General Assistance with your name and effective date
  • Unemployment/workers' compensation statement of benefits with the your name, address, and effective date
