Three Ways to Apply for Lifeline
Apply now to confirm you qualify for the Lifeline program. If you live in Oregon or Texas, visit your state website to apply.

Contact a participating phone or internet company for help with your application.
3. By Mail
Print out an application (English or Spanish) and send your completed application to the address below:

If you have a disability and need assistance with applying, contact the Lifeline Support Center at (800) 234-9473. The Lifeline Support Center can mail you an application if needed.

After you apply, choose a phone or internet company that offers Lifeline and sign up.
