Error Messages Relating to Your Identity

If USAC cannot confirm your information, such as your Social Security number, Identity, or date of birth, we will ask for proof. First, double check that the information you gave us is correct. If it is correct, show us an official, unexpired document. The table below reflects what documentation is acceptable for a Date of Birth (DOB) or last 4 digits of Social Security Number (SSN4) error.

If you receive an “Identity not found” error or if you are identified as a duplicate, you must submit copies of documentation proving:

  1. Your date of birth (DOB) and
  2. The last four digits of your social security (SSN4).

If you are identified as deceased, you must submit copies of documentation proving:

  1. Your date of birth (DOB),
  2. The last four digits of your social security (SSN4), and
  3. You are alive (one from the list below).
    • Current utility bill
    • Current income statement
    • Current mortgage or lease statement
    • Current retirement/pension statement of benefits
    • Notarized letter affirming the identity and alive status

Under 18 Years Old

You must be at least 18 years old to qualify for Lifeline. Only emancipated minors can qualify for Lifeline under the age 18. If you are an emancipated minor, submit a copy of:

  • A court document or certificate that says you are an emancipated minor

If you are over 18 years old, submit a copy of:

  • Your birth certificate that shows you are at least 18 years old
